Statutory Sick Pay record sheet update

Statutory Sick Pay: record sheet (SSP2) has been updated to reflect changes brought in by the Employment Rights Act 2025, to remove the waiting period before SSP can be made.

Employers can use form SSP2 to record details of an employee’s sickness absences for Statutory Sick Pay (SSP) and monitor the use of the 28 weeks entitlement.

The form can now be filled in onscreen. A Welsh translation has been added.

How to complete the form

You need to:

  1. Download and save the form.
  2. Open it using Adobe Reader.
  3. Complete it on-screen.

Statutory Sick Pay: no employee entitlement (SSP1)
An employer uses this form if a sick employee isn’t entitled to SSP or if payments of SSP to an employee end. Your employee will need it to make a claim for Incapacity Benefit.
